Group Intake Form

Information

There is a deposit for planning events, renting gear, and/or taking trip leaders. The deposit will be accounted to your overall fees. Prices may vary. To make reservations, an additional deposit may be required. Deposits can be paid at the Student Recreation and Fitness Center once this form has been received and a staff member has contacted you. No planning will begin without this deposit.
